Nicespeechlady.com has created one document that lists various letters included for SLPs to choose among for using in printing mode for sending with patients to instrumental appointments, or other provider/MDs appointments.
Included are letter forms for:
- Swallowing instrumental assessment appointments – (MBSS/VSA or FEES Sheet from Treating SLP);
- Accompanying requests for swallowing instrumental assessment summary results – (MBSS/VSA or FEES SLP for Treating SLP, Instrumental Testing Results — from the evaluating clinician);
- Esophagram referrals;
- GI referrals;
- ENT referrals; and,
- Other/blank options.
Included in these letters are reasons for referrals, areas of concern, etc. Also, a section for contact information of the referring SLP is available.
Also, Spanish versions of the letters are included, toward the end of the English versions.
